Transcript request form.
The form must be signed by the examinee, and returned to the address or fax
number on the form.
Requests are not accepted online nor via email.
Transcripts are provided free of charge. Please allow up to 2 weeks for
processing.
Can I get a copy of my transcript sent to my school or employer?
Yes, but the request must be signed by you. The request form has an option
for sending copies to a third party.
Can I get a copy of my certificate?
You may or may not have received a certificate along with your original
transcript when you earned your GED. The transcript is the official record of
your GED and can be used as proof for employment, schools, etc. Replacement
certificates are not available.
Can I get the name on my transcript changed?
No, the name of record is the name you provided when you took the tests. The
name cannot be changed.
